Procedure On The Request for Transfer of Source
Schedule of Availability of Service:
Monday to Friday 8:00 a.m. to 4:00 p.m.
Payment is until 4:00 p.m. only with no noon break
Who May Avail of the Services:
Any registered consumers having problem with existing water source.
Requirements:
1. Water bill
2. Letter request
3. Sketch of present & proposed source
Procedure:
1. Present required documents to the Customer Service Clerk and fill up request form for transfer of source.
2. Present documents to the Engineering Department for clearing (as to source).
3. Once cleared, submit documents to Customer Service Clerk for assessment.
4. Pay the investigation fee to any Teller at counters 1 to 5.
5. Present the Official Receipt to the Customer Service Clerk.
6. Get assessment from the Customer Service Clerk after three (3) working days.
7. Pay the total assessment to any teller from counters 1 to 5.
8. Present the official receipt to the Customer Service Clerk for preparation of service request for transfer of source.
